ILLINOIS BONE & JOINT INSTITUTE IMPROVES EFFICIENCY AND REDUCES COSTS WITH PINPOINTCARE About Illinois Bone & Joint Institute Founded in 1990, Illinois Bone and Joint Institute (IBJI) is one of the largest orthopedic group practices in Illinois. With more than 90 physicians in every orthopedic specialty, IBJI offers care for children and adults from a team of accomplished and respected orthopedists. IBJI has 20 locations throughout Chicago and the north suburbs. The Problem Effective January 1, 2014, IBJI elected to participate in the Centers for Medicare and Medicaid Services (CMS) Bundled Payments for Care Improvement (BPCI) Model 3 initiative for major joint replacement (DRGs 469 and 470). This initiative combines payment for the physician, hospital and other post-acute care providers into a single payment, creating an incentive for providers to lower the overall cost of care through efficiency and care coordination. Knee or hip replacement surgery often requires care from multiple providers, including acute care, skilled nursing facility (SNF), home health and physical therapy, which has the potential for fragmented and disjointed care. Additionally, post-acute care is a critical aspect of a patient’s recovery from a major joint replacement. The Solution “ By gathering and sorting patient and surgery specific information that feeds into the Pinpoint platform, IBJI is able to create an individualized care plan which it can use to monitor and subsequently manage cases in a real time environment rather than retrospectively. ” - Andre Blom Director of Rehabilitation Services, IBJI In September 2013, IBJI chose PinpointCare as its platform for patient engagement, care coordination and management of the BPCI initiative. PinpointCare is a cloud-based, flexible software-asa-service (SaaS) solution that facilitates ongoing communication between the care team in settings other than the physician’s office. The goal is to involve patients in the creation of a customized care plan and effectively monitor health status, assess risk and improve time to recovery. With PinpointCare, IBJI is able to coordinate care with 50+ post-acute care partners and contracted providers, including hospitals, SNFs, home health facilities and physical therapy practices, at minimal cost without being on the same EMR. Figure 1 Using PinpointCare, IBJI is able to complete a dynamic, personalized pre-op evaluation for each patient undergoing a joint replacement surgery (Figure 1). This enables better management of care across the continuum for high-risk, potentially costly patients (Figure 2). The Solution (continued) Each post-acute care partner or contracted provider involved in a patient’s care plan is then able to access the PinpointCare system to facilitate patient management throughout the continuum (Figure 3). Since launching PinpointCare, IBJI has experienced improved efficiency and cost reduction in the following areas: REDUCING SNF 6 Reduced costs by an average of 18% per case (per DRG) DAYS Reduced average length of stay for partner SNFs by 6 days LOWERING COSTS 18% LOWER READMITS 14% Lowered costly hospital readmits by 14% Increased patients initiating directly to Home Health by 17% PATIENTS TO HH 17% SAVED OVER 3 $ MIL Saved approximately $1,000 per case since program launch The Future IBJI will migrate to version 2.0 of the PinpointCare solution in early 2015. The revised version will enable improved workflow, additional flexibility to account for unexpected events (e.g. readmissions) and enhanced reporting tools through all phases of the care continuum. IBJI anticipates continued clinical and financial success through this partnership with PinpointCare.
