All receipts for purchases qualifying for rebate programs must be included with the rebate form mailing. Purchase must be made at participating Authorized Contract Distributors, Builder Distributors and Retail Dealers only. Purchases at the following and other retailers are excluded: Best Buy, Lowe’s, Sears and The Great Indoors. See in-store associate for details and to confirm participation. This offer is valid for individual consumer purchases made in the US (except Puerto Rico) and will only be mailed to addresses in US, and offer is void where prohibited, taxed, or restricted by law. Requests with invalid or undeliverable mailing addresses, P.O. Box addresses (except those in North Dakota) or addresses outside of the US will be denied.
